Case Studies

Primary Care based Heart Failure Clinic

Working with a group of GPs to develop, design and implement a primary care based Heart Failure clinic.  This included:

  • Identifying the clinical need
  • Review of existing structure and process of heart failure services
  • Writing and submitting a business case
  • Establishing an implementation plan
  • Establishing an evaluation framework
  • Project management of support to point of implementation
  • Stakeholder management.

Implementation of model of care for Nursing and residential home residents

Working with 26 GP practices to implement a model of care for delivery of primary care and nursing care services to care home residents.  This included:

  • Wide complex stakeholder engagement:
  • Including establishment of six cluster implementation groups
  • Close working with community service providers to align resources
  • Engagement with CCG re future commissioning of services
  • Project management
  • Redesign of initial model of care including increase in staffing allocation
  • Implementation planning and sign off from Board
  • Facilitation and project management support for roll out of model.
